Confidentiality Policy

All notes, records, and communications between you and Kitchener Therapy Collaborative concerning your involvement as a client are kept secure and confidential and can be shared only with your written consent. There will be a record of all appointments/visits.

Any client information is kept within our practice in the strictest confidence.

 

There are times when you, the client, may want information shared with others, such as WSIB, physicians, workplaces, insurance companies, schools, families, etc. In this case, you will be asked to sign a Release of Information form before any information is shared. We believe in collaboration, therefore, we will work with you and your support system when/if you ask us to do so.  


There are a few exceptions to confidentiality. Under specific conditions, the law may require that clinical records and/or information be disclosed. These situations include:


1.         If you are at imminent risk to harm yourself or another person;
2.         If there is suspected abuse, including the neglect of a child under the age of 18;
3.         If you disclose information pertaining to assault or abuse by a health professional; or

4.         If there has been a court order or a subpoena.
 

*When appropriate, and with the client’s best interests in mind, your therapist may consult with their supervisor or another practitioner at KTC regarding an aspect of your situation. Every effort will be made to protect your privacy and ensure that no identifying factors about you are shared. We will adhere to the strictest confidentiality guidelines, as is outlined by our professional regulatory body.
